If it is the coop not launching for the first time make sure you are both using the same DX version in game, make sure there are absolutely no mods, make sure no one is overclocking and also for arguments sake check NAT types and Antivirus. If you already started a coop campaign and this occurred when you tried to reload it or a few turns in then the host needs to send there version of the save game to the other player and they need to replace there save with the hosts. No mods or else both running the same mod pack with a multiplayer compatible version install. You need to be running the games with the same DX version (you can change this in settings).
